Black Bolt needs to vocalize to activate his brain’s ability. With his mouth sealed, there was no way for the blast to escape. 

Could it exit through his nose? Biologically and neurologically, no. 

The nose’s primary function is breathing, not connected to the brain’s speech function, preventing the particle-electron storm from escaping that way. 

Thus, lacking an exit for his power, as you notice in the movie scene, the blast imploded within his skull instead of escaping through his mouth.
